Community Health Facilitator JOB Overview:
Enhance the patient experience through facilitation/coordination of care, informing and educating patient on community resources and striving to improve continuously enhance and improve the global patient experience.

AREA OF ASSIGNMENT:
Clinic Network RESPONSIBLE TO:
Clinic Manager PREREQUISITES:
High school graduate or General Education Diploma (GED) preferred.
Minimum 2 years experience in an ambulatory care setting.
Good to excellent computer skills including the ability to successfully navigate between multiple applications.
Preferred Medical Terminology Personable, excellent communication skills.
Care Coordination or Patient Navigator training completed within 6 months of employment.

Enhance the patient care experience Engage the patient in care Empower the patient with knowledge and access to support resources Improve health outcomes for patients Improve the patient and family experience of care Improve primary care team satisfaction Participate in team change package, measures and associated collaborative goals and objectives Provide information and access to supportive counseling as appropriate including the following:
Crisis intervention Referrals to internal and external resources Contact relevant community agencies to increase knowledge of services available for patients and families.
Educate per protocol When indicated, provide assistance with/advocate for patients/families.
Provide accurate information to patients, physicians, staff and community as requested in a timely fashion Take responsibility for professional development to maintain and increase competency and support the education of patients, families and staff:
Participate in ongoing education relevant to role Provide guidance to staff for dealing with difficult patient and family situation Establish contact with community resources and develop tools to educate peers on patient advocacy and support networks Maintain confidentiality of patient information and medical records.
Demonstrate awareness of the importance of cost containment for the department.
